Introduction

Evolutionary Clinic fields constant patient questions and booking requests, often outside clinic hours. Front-desk staff spent much of the day answering the
same questions and managing the calendar, leaving less time for care. Monolythic Tech built a RAG-grounded AI assistant that answers patient questions
from the clinic's own documents and handles appointment booking, with a clean handoff to staff whenever a human is needed.

Project Goals

  • Answer common patient questions accurately, any time of day

  • Let patients book, reschedule, and cancel without calling

Challenges

  1. Accuracy: health answers must come from approved information, never invented

  2. Sensitivity: the assistant had to know when to step back and involve a person

  3. Scheduling: real availability, multiple practitioners, and clinic rules

  4. Accessibility: patients of all comfort levels needed a simple experience

  5. Privacy: patient information had to be handled carefully and securely

Process

  • Research & Discovery: reviewed the clinic's most common enquiries, booking patterns, and the questions staff repeated daily.

  • Workflow Mapping: mapped which requests the assistant should fully handle, assist with, or escalate.

  • Agent & Retrieval Design: built a RAG layer over clinic documents so answers were grounded, current, and on-policy.

  • Build & Integration: connected the assistant to the booking system and messaging channels via n8n and an LLM agent.

  • Evaluation & Hardening: used Langfuse to evaluate answer quality and trace edge cases, adding guardrails and human handoff before launch.
